Vacancy Type: Work Experience: Age: Duties Community nurse provide valuable services to the public, patients who are the target children and their families. These health care professionals single out medically related needs in the local community and take part in outreach clinics, as well as one-on-one visits with patients: Home Visits - A community nurse performs visits to home-bound patients. This allows the nurse to get to know a patient's condition and health history and aids in determining a plan of care. Creating Nursing Plans - With the information gleaned from home visits, a community health nurse compiles a plan that outlines the course of treatment for the patient. He or she utilizes data from the home visit and the doctor's treatment plan to shape the patient's recovery needs. Monitoring Communicable Diseases - A community nurse keeps close watch on the communicable diseases spreading around the community areas and organizes with doctors and community members in order to develop programs that offer information and advice on these diseases. Instituting Public Programs - The health care needs of the community are at the forefront of a community health nurse's profession. The nurse identifies the illnesses that provide the most threat to the community and takes part in educating the public on how to avoid and treat these illnesses. Health Educations - A community nurse can improve the health of families and communities through teaching them about healthy lifestyle choices. Educating the public on eating healthy, the basics of looking after their own health and other information is one of the primary responsibilities of the community nurse. It's the means by which the nurse can help prevent illness that plagues the community he serves. - Evaluations are also helpful to improve preventive strategies, as well as determining the health trends of the population. The nurse can use evaluations to determine the health priorities for a given area. Community Builder - A community nurse has a role to play in helping to build the community she serves. A healthy community is a strengthened community, and a competent and compassionate community nurse plays a vital role. To that end, a community nurse is responsible for building relationships and partnerships throughout the community, in order to improve the health of individuals and families. As someone who seeks to help strengthen and build the community around her, the nurse acts as an advocate as well. As an advocate, nurses can ask for additional resources to help the community and make others aware of the health concerns of that community. - Performing other duties as required

- Cambodian national. - To do this, you must have either an associate degree in nursing (ADN) or a Bachelor of Science degree in nursing (BSN). - Good organizational skills and honesty. - Able to use computer software (MS-Word, MS-Excel, MS-PowerPoint, Internet and E-mail). - Demonstrate commitment to the values and mission of the Pathways to Development. - Excellent report writing and verbal communication skills. - Demonstrate ability to work collaboratively and effectively with colleagues and community-based organizations.
